It makes the file's digital fingerprint (SHA-256) and asks VirusTotal whether this file has been checked before. If it has, the bot knows the answer right away, without uploading your file.

Check that the group or channel has been approved and that the bot is an admin. In a channel it also needs the Post Messages right. Photos and voice messages aren't checked.
